<p><strong>Q. What is glaucoma, and how many people does it affect?</strong><br />
A. Glaucoma is a vision-threatening eye disorder that usually occurs when excessive pressure inside the eye damages the retina, optic nerve or both. Glaucoma affects 3 million Americans, and about 120,000 are legally blind because of it. Left untreated, glaucoma can lead to blindness in three to 15 years, depending on its severity.<span id="more-25"></span></p>
<p><strong>Q. Who is at greatest risk?</strong><br />
A. Glaucoma most often occurs in people over age 40. People with a family history of glaucoma and those who are very nearsighted or diabetic also are at a higher risk. Research shows black Americans are about seven times more likely to develop glaucoma. It is the leading cause of blindness among black Americans. Hispanics also face an elevated risk.</p>
<p><strong>Q. What causes glaucoma?</strong><br />
A. Your eyes produce an internal fluid designed to help the eyeball keep its shape. This fluid is constantly draining out of the eye. If too much fluid is made or if the &#8220;drain&#8221; backs up, which is usually what happens in glaucoma, pressure begins to build inside the eye like air in an overinflated tire. The extra pressure damages sensitive nerve tissues. The result in most cases is chronic glaucoma that slowly worsens over time. There&#8217;s also a type of glaucoma in which damage occurs without elevated pressure, but we don&#8217;t often see this low-pressure glaucoma.</p>
<p><strong>Q. What are the symptoms of glaucoma?</strong><br />
A. Usually the first stages have no symptoms. That&#8217;s why it&#8217;s so important to get a yearly eye exam to detect the condition early. A glaucoma &#8220;puff test&#8221; is a standard part of a comprehensive eye exam. There&#8217;s also an acute form of glaucoma in which the &#8220;drain&#8221; shuts down rapidly. This condition is rare but extremely painful, so there&#8217;s no mistaking it. Acute glaucoma requires immediate attention in a hospital emergency room.</p>
<p><strong>Q. Is there hope for glaucoma patients?</strong><br />
A. We&#8217;ve seen the development of several powerful new medications that can be quite effective at slowing down or even halting the progression of glaucoma. These new medicines often are used along with laser surgery to improve drainage in the eye and thus reduce pressure. Glaucoma often is very treatable with early detection.</p>
<p><strong>Q. How do your patients feel when you detect their glaucoma early and save their eyesight with treatment?</strong><br />
A. Many older patients have told me how relieved they are that they still have good vision after many years of glaucoma. For example, a good friend told me a few years ago her eyes felt funny. I gave her a thorough eye exam, found she had low-pressure glaucoma and sent her to a specialist. She hasn&#8217;t lost any additional vision at all, and her eyesight has improved thanks to medication and surgery.</p>

<p>&#8220;Today, there&#8217;s increasing evidence that ultraviolet rays in sunlight can significantly increase your risk of developing cataracts and macular degeneration.</p>
<p>&#8220;Because both conditions can threaten healthy vision, I recommend wearing sunglasses on sunny days year-round. Also, you&#8217;ll experience better night vision when you wear sunglasses on sunny days.&#8221;</p>
<p>What&#8217;s the best way to make sure your new shades provide maximum vision protection? Says Dr. Cohen: &#8220;When buying sunglasses, make sure they carry a consumer-protection label stating they&#8217;re 99 or 100 percent UV absorbent, or provide UV absorption up to 400 nanometers (nm).</p>
<p>&#8220;Once you&#8217;ve eliminated the threat from UV light, you can focus on other issues such as reducing glare and choosing a tint that will allow you to control the degree of brightness reaching your eyes.&#8221;</p>
<p>Here are tips for making sure your new sunglasses protect your vision:</p>
<ul>
<li>Larger lenses and wrap around sunglasses provide added UV protection.</li>
<li>Polarized lenses are designed to block out sunlight glare that bounces off windshields, pavement and other smooth surfaces.</li>
<li>Gray lenses are optimal because they don&#8217;t modify colors. Green and brown also are good choices.</li>
<li>If you often wear corrective lenses outdoors, consider prescription sunglasses that protect against UV rays and glare <em>and</em> enhance your vision.</li>
<li>To convert ordinary corrective eyeglasses to sunglasses, try clip-on tinted lenses or tinted lenses that attach magnetically to the frames of prescription glasses.</li>
<li>Consider wearing photochromic glasses that darken automatically outdoors, then lighten again when you go back inside.</li>
</ul>
<p>Once you&#8217;re sure your sunglasses will protect your eyes, select a style you enjoy most. &#8220;Walking on the beach in your new shades is lots of fun,&#8221; says Dr. Cohen. &#8220;You&#8217;ll enjoy it even more knowing you&#8217;re also taking good care of your eyes!&#8221;</p>

<p>&#8220;I&#8217;ve been providing eye exams for more than 12 years now,&#8221; says the optometrist from Norfolk, Virginia. &#8220;During that time, I&#8217;ve diagnosed four different tumors and several cases of diabetes in patients who didn&#8217;t even know they had a health problem.<span id="more-6"></span></p>
<p>&#8220;As you might imagine, I encourage everyone to get an annual eye exam — not just an eye screening!&#8221;</p>
<p>Dr. DeVito stresses that there&#8217;s a big difference between eye exams and eye screenings.</p>
<p>&#8220;Screenings are partial, limited eye assessments that take place in locations other than an eye doctor&#8217;s office,&#8221; he says. &#8220;Typically, a screening will be performed by a nurse at school, a pediatrician in the office or maybe even a clerk giving motorists a vision test for a driver&#8217;s license.</p>
<p>&#8220;There&#8217;s no doubt that screenings can be helpful at times in detecting some problems with vision, but I encourage patients to have a thorough eye exam in an optometrist&#8217;s or ophthalmologist&#8217;s office,&#8221; Dr. DeVito says. &#8220;The doctor can take an entire eyecare history and patients can take advantage of the doctor&#8217;s on-site diagnostic and treatment tools.&#8221;</p>
<p>Dr. DeVito recently installed a state-of-the-art instrument that detects glaucoma by examining tiny fibers in a patient&#8217;s optic nerve. &#8220;This new equipment allows us to spot damage in nerve fibers long before it would show up in the traditional eyepressure test.</p>
<p>&#8220;Early detection is crucial with glaucoma, and this test is only available during a thorough eye exam in the eye doctor&#8217;s office. You won&#8217;t get it during a screening.&#8221;</p>
<p>According to Dr. DeVito, annual eye exams also provide other benefits to patients.  The eyecare doctor can:</p>
<ul>
<li>Measure for prescription lenses to correct nearsightedness, farsightedness        and astigmatism.</li>
<li>Check for the presence of eye diseases and conditions such as glaucoma, macular degeneration, cataracts and diabetic retinopathy.</li>
<li>Make sure your eyes are working well together, while also evaluating your eyes as part of your overall health.</li>
</ul>
<p>&#8220;In recent years, I&#8217;ve had several situations in which I detected the presence of blood in the back of the eye, which is one of the first signs of diabetes,&#8221; says Dr. DeVito. &#8220;Those patients first learned they had a serious health condition during a routine eye exam in my office. I think that&#8217;s a great example of how worthwhile an annual eye exam can be.&#8221;</p>
